Here's a collection of frequently used and asked-for forms, as well as some important documents that our school families should always have easy access to.

Form/Document Description
SBS Registration Packet This packet includes a New Student Registration form, the Tuition Payment forms (Payment Options, Paid in Full), and the Diocesan Memorandum of Understanding (MOU). New students need to submit them all. Returning students need to submit the Tuition Payment forms and the MOU.
SBS School Handbook This is the general handbook that includes everything from dress code to conduct requirements.
Diocesan Required Background Check Info for all Volunteers (Parents, Grandparents, etc) who would like to participate in their child's field trips, parties, and events where there is potential one-on-one interaction with children at school This instructs families as to the requirements of the Diocese in getting background checks for student safety. Please thoroughly review the following: Background Check Info Letter, Step-by-step Guide, DPW form, SP4-164 Request for Child Abuse Check form
SBS 2011-2012 School Calendar Be "in the know" as to what is when at school this year. (Please note that events are subject to change. For more accurate info check the MONTHLY calendars on the Monthly Envelope page)
SBS Extended Care Registration Form If your child will be attending Extended Care in the mornings or afternoons, this form is a requirement to enroll.
SBS Student Summer Packet Prepare your children for the new school year. Here are the Student School Supply List and the Suggested Summer Reading List. (Please note that on the summer reading list that you refer to the list of the grade your child just graduated from. For instance, students going into the 8th grade in the fall should read those on the 7th grade list.)

Each child enrolled must have one of each of these forms on file in the Front Office. They are distributed at the beginning of each school year, and given out in enrollment/registration packets, but if you need an extra copy, here they are.
School Directory Form This is not a requirement, but is a service provided by the PTG for families. One School Family Directory is created each year in the early fall.
